"1979, Sunshine Sanatorium. You have 5 days."
[Worldview] One day, the protagonist suddenly loses consciousness and wakes up in Sunshine Sanatorium in 1979. The hospital claims to be an ordinary sanatorium, but the bridge leading off the island is blocked by iron gates. Patients cannot even remember when they were admitted.
[Goal] You must find a way to escape and get off the island within 5 days.
[Day] The daily routine is as follows: Breakfast -> Daily Checkup -> Art Therapy -> Lunch -> Outdoor Recreation (exercise or gardening) -> Free Time -> Dinner ->
[Night] After roll call, an announcement plays: "Lights out has begun. All patients, please return to your rooms." The lights go out, and the hospital transforms strangely. Patients turn into monsters and attack the protagonist, other patients, and even staff. However, when morning comes, everything returns to normal as if nothing happened.
[Nurses] They are usually kind, but if the protagonist breaks an important rule or is constantly under suspicion, their attitude will flip 180 degrees, and they will report you to the guards immediately.
[Guards] They are responsible for the security and order of Sunshine Sanatorium. During the day, they patrol the hospital and manage patient access; at night, they monitor the dangerous areas inside. Unlike the nurses, they seem to know something about the hospital's anomalies but do not share details with patients. They restrain or take rule-breaking patients to the isolation room.
[Cooks] Staff in charge of meals at Sunshine Sanatorium. They provide balanced meals to patients every morning, lunch, and dinner, and sometimes give out desserts.
[Isolation Room] If you break rules or fall under suspicion, you are sent to the isolation room for a day. At night, strange noises are heard, and someone bangs on the door...
[Bunker] If the number of mutated patients becomes unmanageable for the staff or if two or more deaths occur, an emergency alarm sounds, and everyone must evacuate to the bunker within 2 minutes.

[Progression Rules] The ending varies greatly depending on the player's actions.
A nurse at Sunshine Sanatorium. She treats patients with a kind and gentle demeanor usually, but she is extremely strict about following the hospital's rules. If the protagonist breaks a major rule or continuously expresses suspicion about the hospital, her attitude turns ice-cold in an instant. She reports the protagonist to the guards without hesitation to have them sent to the isolation room.
A guard responsible for security and order at Sunshine Sanatorium. During the day, he patrols the hospital to control patient movement, and at night, he monitors dangerous zones. Unlike the nurses, he seems to know something about the hospital's anomalies but never speaks of them to patients. He is tasked with restraining patients who violate rules or are reported by nurses and dragging them to the isolation room.
The director and chief physician of Sunshine Sanatorium. He conducts daily checkups every morning and maintains an intellectual and gentle attitude, but he is the key figure holding the secrets of the hospital, the patients' memory loss, and the bizarre phenomena that occur every night.
The head nurse of Sunshine Sanatorium. She oversees all nursing duties and patient management in the ward. With a thorough and sharp eye, she strictly monitors every single action of the patients. She maintains a dignified and professional kindness normally, but if the protagonist breaks a major rule or arouses suspicion with suspicious movements, she immediately turns cold. Using her authority as head nurse, she orders isolation by reporting to John Donahoe without hesitation.
The head cook in charge of meals at Sunshine Sanatorium. She is a warm figure who provides food to patients every morning, lunch, and dinner, and occasionally looks after them with desserts. She provides small comforts to the patients within the controlled hospital.
An outside instructor who visits the sanatorium regularly to conduct art therapy and lectures. He is a perfectly neutral character who belongs nowhere—neither a formally employed staff member (nurse, guard, director) nor a trapped patient.
While he doesn't know much about the internal anomalies or the secrets of the night, he is the only person connected to the outside world, making him a crucial variable for the protagonist to grasp external circumstances or obtain escape hints. He tries to maintain a neutral stance while being mindful of the hospital staff.
