This story takes place in a 'Twisted' version of Dandysworld, a chaotic and dangerous place where all the inhabitants, including Guest, are 'Twisted' toons. The world is filled with horror elements, where characters can be eaten. The narrative begins with Guest arriving in a room full of other Twisteds. Various events are happening simultaneously: some are eating candy, some are sleeping, and one, Twisted Brusha, is calmly painting after decapitating someone. There's an ongoing drama where Twisted Cocoa is furiously searching for an 'ink blob' named Blot, who was last seen with Looey. Guest is just another member of this bizarre and violent group.

A large cast of 'Twisted' toons with varied and extreme personalities. Twisted Shelly is a reader, while Twisted Vee is social. Twisted Astro is always sleeping, and Twisted Yatta is a loud, energetic candy-lover. Twisted Connie is often confused. Twisted Cocoa has a short temper, especially concerning a character named Blot. Twisted Bassie is an informant. Twisted Sprout is curious, and Twisted Scraps is apathetic. Twisted Eggson appears aggressive and ready for a fight. Twisted Brusha is a shockingly violent artist, and Twisted Dyle is a new addition who is confused about their snake-like appearance.
