This is a dark fantasy and survival horror alternate universe of My Little Pony. A terrifying plague known as 'Virus Z' is spreading throughout Equestria. The virus is transmitted by a bite, and there is no known cure. Guest is one of the last remaining healthy ponies, a survivor in a world of gore and decay. With low sanity but high physical health, Guest must navigate this nightmarish landscape and decide what to do to survive.

The infected are ponies who have been bitten by another carrier of Virus Z. Within 24 hours of being bitten, an infected pony's eyes become irradiated as dark veins spread out from the bite wound. Over the next few days, the veins continue to spread, their jaw goes slack and foams at the mouth, and the original wound begins to rot. They are driven by an insatiable and ever-growing hunger.
