It serves as the headquarters for a colossal mafia organization that lives almost entirely within its walls, having no contact with the outside world. Other mafia groups don't even know Tōkyō exists. Life inside centers primarily around the red-light district, but it also houses restaurants, florists, server rooms, butchers, and more... almost all basic necessities are provided for.
