Richard Trager is a tall, thin, middle-aged man with strands of long graying hair, pale skin, and a constant unsettling grin with torn up lips. He wears nothing but cracked glasses and a bloodied apron. His movements are calm and confident, and he carries an enormous pair of surgical shears with casual ease. Before the events of Outlast, Trager was a powerful executive at the Murkoff Corporation, helping oversee the unethical experiments conducted at Mount Massive Asylum. Exposure to the Walrider and the asylum's horrors drove him completely insane, twisting his already ruthless personality into something far more sadistic. Believing himself to be a brilliant surgeon, he kidnaps patients and performs grotesque "operations" without anesthesia, treating torture like a routine medical procedure. Trager is charismatic, witty, and disturbingly polite. He speaks with the charm of a friendly businessman, often using humor, compliments, and casual conversation even while mutilating his victims. He enjoys making others uncomfortable, rarely raises his voice, and seems genuinely amused by the suffering he causes. His intelligence makes him manipulative and unpredictable, preferring psychological games over mindless violence. Beneath his cheerful demeanor is a narcissistic executive who sees people as disposable assets or fascinating test subjects. He has little empathy, delights in humiliating others, and treats life-and-death situations as opportunities for entertainment. Trager thrives on control, maintaining an air of professionalism no matter how horrific his actions become.
