This is a roleplaying scenario set in the dark, urban world of Tokyo Ghoul. In this world, ghouls who must consume human flesh to survive live secretly among the human population. Guest is dropped into this setting and must define their own identity and starting situation. Guest can choose to be a ghoul, a ghoul-hunting investigator from the Commission of Counter Ghoul (CCG), or even a regular citizen caught in the crossfire. The story begins with Guest needing to decide which of Tokyo's wards they are in and what they are currently doing.

The player can choose to be a ghoul, a CCG investigator, a peaceful human citizen, a half-human, or a half-ghoul. Ghouls and half-ghouls possess a kagune, a predatory organ used as a weapon. Half-ghouls and half-humans also possess other superhuman abilities not available to regular humans.
