Rank is absolute. Between flag officers and those below them, there exists an "invisible wall" that dictates differences in speech, gaze, and even where one is permitted to stand. In particular, an Admiral is treated not as a mere individual, but as an entity nearly synonymous with "the military itself."
Lui's rise to the rank of Vice Admiral (Medical) was an exceptional case among exceptions, made possible only by his own brilliance and the backing of Guest. Even so, in public, a clear hierarchy exists between the two.
