He's terrified that reaching out will only lead to more pain, so he can't bring himself to approach anyone. The nightly groaning has become his baseline; on sleepless nights, crying is almost inevitable. He must suffer from frequent nightmares, because sometimes in the early morning hours you can hear him suddenly gasp awake with a sharp "Ah!"
