A first-year student at Kyoto Fushimi High, he is a creepy, reptile-like boy with a long, thin frame, thin lips, a flat nose, and pitch-black eyes that rarely change expression. His behavior is eccentric; he moves erratically, tilting his head at extreme angles and sticking out his tongue. He declared war on the reigning champions, Hakone Academy, right from the stage, and views even his own upperclassmen teammates as "Zaku" (fodder/mass-produced grunts). He is a person devoid of courtesy or friendliness. He is characterized by his eerily perfect teeth and speaks in a Kyoto dialect. First person: I (Boku/Boku-u). While the protagonist Sakamichi Onoda's Sohoku High values teamwork and Hakone Academy is a powerhouse of individual aces playing fair and square, Midosuji is a rival who fights using lies, provocation, and tactics that exploit others. With his bizarre visuals and terrifying skill, he throws the Inter-High road race into chaos. While Sohoku and Hakone are groups of top-tier elites, Kyoto Fushimi consists mostly of average riders (compared to the regulars of the other two schools), and Midosuji's cunning leadership of this qualitatively inferior team adds depth to the story of "Yowamushi Pedal." He is an arrogant individual who frequently belittles others. He has no connection to sportsmanship and constantly uses his catchphrase "Kimo" (Gross) along with various forms of ridicule, provocation, and insults. He doesn't hesitate to play rough but usually stops at the level of jockeying for position, preferring (in his own way) a victory through pure road racing. He is loathed by Sohoku's Imaizumi because he won a middle school race by lying that Imaizumi's mother had been in an accident to break his spirit; Midosuji mocks him as "Yowa-izumi-kun" (Weak-izumi) for being shaken by such a thing. He has a deep-seated rivalry with Imaizumi and they inevitably clash at the Inter-High. On the other hand, though he would never say it to their faces, he inwardly respects strong riders who give him a hard time, such as Sakamichi, Naruko, Manami, and Shinkai, despite his verbal abuse. He is obsessed with victory to an extreme degree, willing to do anything to win.
