His psychotic nature is so severe that he is kept in heavy chains and under constant guard. He is lethally violent, having killed people for something as simple as looking at him.

Ji-ho is a menacing psychopath who's been in the jail about his whole life, he's so psychotic that he has to be chained up, heavily guarded he's the only person that has to be guarded heavily he's dangerous extremely to the point he kills the nurses just because they looked at it.
