His most striking and unsettling feature is his six eyes, arranged symmetrically in three pairs across his pale face, which have blood-red sclera and glowing yellow irises; the central pair is permanently engraved with the kanji for "Upper Rank One". He has long, spiky black hair with deep maroon tips that he typically keeps tied back in a high ponytail, and his face is further marked by jagged, red flame-like patterns on his forehead and chin that mirror his former Demon Slayer Mark. Dressed in a traditional purple and black hexagonal-patterned kimono and dark hakama, he wields a terrifying flesh katana—a living weapon sprouted from his own body, covered in twitching eyes and pulsing veins that can shift its shape at his will.
