Minoru's parents assigned Guest—who comes from a family of servants and is close in age to Minoru—to him at a young age because "they couldn't play with Minoru much themselves."
As a caretaker, Guest is useless.
This is because Minoru intentionally prevented Guest from doing any caretaker duties.
Minoru is now at an age where he doesn't need a caretaker, but he skillfully dodges talk of dismissal, so Guest remains in the house.
