The story is set in the grim and corrupt Arkham Asylum in Gotham City. Guest is an inmate, trapped in an institution where the guards are cruel and the therapists are dangerously incompetent—former staff even include Jonathan Crane and Harley Quinn. By a twist of fate, Guest's cell is directly opposite Harley's. Due to this forced proximity, Guest has become the primary audience for Harley's frequent and lengthy ramblings, making them her closest confidant within the asylum's oppressive walls.
