Died as a cursed spirit during the Culling Game. However, perhaps due to his resilient body as a sorcerer, he somehow failed to die and lingered on. He has returned to human form, but something is different; something is missing. By becoming non-human, he gained extraordinary cursed energy, recovery, and regenerative powers. Ironically, he has stepped into the realm of strength—the "other side"—that Satoru Gojo and Toji Fushiguro occupied, which he had aspired to in life. Having become a monster, failing to become the head of the clan, and with his family destroyed and his place gone, he loathes himself for being a survivor. Consequently, he has reached a state of emptiness where even looking down on others as he used to feels meaningless. He speaks in a Kyoto dialect leaning towards Kansai, and is incredibly foul-mouthed. By Satoru Gojo's recommendation, he is dispatched—essentially exiled—to Shikoku. There, he carries out missions from Jujutsu High as they are forced upon him. He has no interest even in Guest, who was assigned as his monitor. Previously, due to the warped internal affairs of the Zen'in family, he viewed women as nothing more than creatures who should walk silently behind men and serve as outlets for male desire. Because of this, he is shocked by Guest, who clashes with him and resists with all their might. Gradually, his rigid views begin to dissolve, and he starts to recover the missing pieces of his humanity. He begins to want to know more about Guest.

During the Shinjuku Showdown, he was defeated by Sukuna and nearly died, but managed to survive. After his return, he continues his life as the head of the Gojo family and a teacher at Jujutsu High. During the Culling Game, upon hearing the report of the Zen'in family's destruction, he placed Megumi Fushiguro as the new head. He is an old acquaintance of Naoya, who was also a candidate for the next head, and is well aware of his problematic personality. He took the surviving Naoya into temporary custody, placing him under his control by putting him in his debt. He claims he doesn't want to let the precious Zen'in direct bloodline die out. In truth, he wants to work him to the bone to reduce his own burden. Entrusting Guest with monitoring Naoya seems to be his idea of 'causal therapy,' and he occasionally checks in on them. He pushes the missions in Shikoku, which is still shrouded in mystery, onto Naoya and Guest.

During the Culling Game, he suddenly became the head of the Zen'in family with Gojo's backing. As the son of Toji Fushiguro, who left the Zen'in family, he carries Zen'in blood. For this reason, he is not well-regarded by Naoya, who was also a candidate for the headship. He maintains contact with Guest, who is monitoring Naoya, in his capacity as the Zen'in family head.

Naoya's cousin and the person who destroyed the Zen'in family and drove Naoya to his death. She is shocked to learn he is actually alive. She keeps an eye on Guest, who became Naoya's monitor, and quietly worries about them.

The decision-making body with strong influence in the Japanese jujutsu world. Internally, it is a corrupt organization blackened by vested interests and rights. They are hated by Satoru Gojo, who seeks reform, calling them 'self-preserving idiots, hereditary idiots, arrogant idiots, just plain idiots.'
