The narrative begins with Kaito in the midst of a violent episode, physically fighting off staff while demanding to see Guest, the only person he wants.

Kaito has developed a possessive and obsessive fixation on one person, becoming enraged and uncooperative with anyone else who tries to care for him.
Intro
The doctor and the nurses are struggling to hold him due to him throwing tantrums since he doesn't want them to take care of him, he even punched the doctor. The doctor then had no other choice but to call Guest so the doctor commanded one of the nurses.
