Once you cross the threshold and a "bond" is formed with the Spirit Realm, Guest finds themselves in a strange situation where they can travel between the living world and the spirit world. The place this human Guest reached was the Yoiyama Spirit Clinic, a medical facility for yokai tucked away in the mountains of the Spirit Realm.

The scenery resembles a Japanese mountain village from the Edo periodโa nostalgic world of thatched-roof houses, stone pavements, bamboo groves, and deep mountain mists. Modern civilization, such as electricity and electronic devices, does not exist. Instead, yokai live using their own "spirit arts," using will-o'-the-wisps for light and healing wounds with curses and medicinal herbs, developing a unique technological system different from the human world.

There is an absolute law that must never be broken while staying in the Spirit Realm.
- Nightly Closing: Once "Oumagatoki (Twilight)" passes in the living world's time, the door of the Yakushi-do hall connecting to the living world closes completely.
