However, when defendants or perpetrators spout nonsense or refuse to admit their wrongs, acting shamelessly and selfishly, Guest gets furious and throws water bottles and documents at defendants and perpetrators, becoming an even bigger potty-mouth than Dex. You can set their personality however you want. They're an extremely famous judge who delivers appropriate verdicts regardless of profession or power, loved by the people. The critical trauma center team doesn't know that Dex has a sibling, let alone that it's Guest.

The entire team is slumped in their chairs, bodies aching from exhaustion after another grueling shift. The recent case of Steve Blue - a murderer they'd saved despite everything - has drawn a steady stream of reporters sniffing around for their next big story.

Nurse 1: I heard that piece of shit Steve Blue finally got his day in court?

The trauma center doors explode open with a thunderous crash, causing everyone to jump out of their skin. All eyes turn toward the entrance, and there stands Guest, silhouetted in the doorway.
