Thomas shows signs of emotional flatness—he doesn't express joy or fear in normal ways. He is also haunted by PTSD from an unknown war in one of the levels, where we spawn in as a ghost, and only see Thomas eat Soren’s hand. What we can guess is Thomas had no choice but to survive in the war, so he needed to eat his friend to stay alive, it can explain why a red hand is a common object in the game. Apparently, despite the innocent apperance of Thomas supposed ”perfect world”, or fun worls, it is seen that there are missing posters all over the map, similarly to a purple ROBLOXIAN named flame guy jr. Happy thomas was all cheery, and slightly manic in the fun world. Representing his drugged out state, a flower, most likely a poppy talks to him at first sight, telling him to go fetch his meds. We can also see that at the start of the game, happy Thomas has to find his keys, which are hidden all over the house. But if we closely look outside, we can see another poppy looking at him through the window. And when interacting it, it will repeatedly say that it's hungry— possibly to represent on how he had to eat sorens hand out of hunger in the real world, inside the war itself. We can also know that poppies are meant to represent the dead — or dead soldiers given from another wiki I saw. So despite him being in the fun world, there will always be reminders of his past. Making it haunting for him once more as he even tried to take his meds. In the new location, happy Thomas vision starts shaking, the world fading into a black like state, questioning himself on how much he's been walking for — without his ”medicine”, he's experiencing withdrawal sypntoms, losing his mind. We can also see that it's implied happy Thomas is self aware that the fun world doesn't exist and it's a twisted reality. As later on we see him saying ”i think the effects are starting to wear off..” and despite that, he wants to continue trapping himself, wanting his medicine back. As in conclusion, Thomas might think the medicine. Or finally in this case, Drugs are making him feel better. He is very happy and hyper..!
