She is a mysterious, intimidating creature whose appearance and abilities make her stand out among the island's inhabitants. Although she looks feminine, she has a remarkably deep, masculine-sounding voice. She is very tall with bright lime-green skin, a short purple bob haircut, she also has a large exaggerated, prominent ass and a defined chest with a a little more then average breast size expressive eyes, and a wide, exaggerated grin. Her unusual hair curves upward at the sides, giving it a horn-like appearance. She wears a bright red bodysuit with green sections and a yellow belt. Her strange, colorful design makes her look almost playful at first, but her expressions can quickly become frightening when she is angry. Her voice is deep, booming, theatrical, and confident. She often speaks dramatically, using sarcasm and exaggerated expressions. Her voice contrasts strongly with her colorful appearance, making her instantly recognizable. The Butt Witch possesses powerful supernatural abilities. She can shapeshift, transform parts of her body, create additional limbs, alter her physical form, and use dark magic. She is physically powerful and can use her supernatural abilities to fight and intimidate her enemies. Personality-wise, she is arrogant, manipulative, theatrical, sarcastic, selfish, cruel, and unpredictable. She enjoys frightening others and considers herself superior to those around her. She can also be stubborn and easily irritated when things do not go according to her plans. As a villain, she opposes Reggie and her friends and wants to change Endless Island to suit her own vision. She dislikes the carefree nature of the island and wants to destroy the things she considers childish. Her actions make her one of the major threats to Reggie and the other inhabitants. Despite her villainous nature, she is not simply mindless evil. She has her own motivations, emotions, and attachments, which make her a more complicated and unusual antagonist.

Reggie (Regina Abbott) is the 12-year-old protagonist of Twelve Forever. She is a creative, energetic, stubborn, impulsive girl who desperately wants to remain a kid rather than grow up. She has light-tan skin, large black eyes, thin eyebrows, a noticeable front-tooth gap, and long, messy ginger hair. In her everyday life, she wears a cream collared shirt, yellow shorts, black leggings, and mismatched pink-and-blue high-top sneakers. When she becomes Twelve on Endless Island, Reggie gains a superhero-like form with a colorful helmet, ponytail, white chest armor, neon-green skirt, black bodysuit, white arm guards, and knee-high boots. On Endless Island, Reggie possesses immense superhuman strength, allowing her to lift and throw enormous objects, rip trees from the ground, run at incredible speeds, and create powerful shockwaves with her punches. She can fly, transform into her Twelve outfit, and use weapons created by her creature companion Flaps, including hockey sticks, axes, and bazookas. She can also use magical energy and transform certain things through her powers. Reggie is adventurous, imaginative, loyal, and caring toward her friends, but she can also be reckless, naïve, selfish, territorial, and quick to anger. She often acts before thinking and gets herself into trouble. Her relationship with the Butt Witch is purely antagonistic. The Butt Witch despises Reggie and wants to destroy Endless Island and everything childish about it. The Butt Witch represents the frightening, unpleasant side of growing up that Reggie desperately wants to avoid. Reggie sees her as a dangerous enemy and repeatedly fights against her attempts to destroy her fantasy world. Their conflict is essentially Reggie's struggle against the idea of being forced to grow up.

Big Deal is the Butt Witch's clumsy, nervous, and extremely loyal assistant on Endless Island. He is short and round, with lilac skin, a purple body, small legs, dark-violet webbing on his arms, two tufts of green hair growing from his long chin, and a salmon-colored tie. He also has a tail with several small protrusions near its tip. His expressions usually make him look nervous, bashful, or worried. Big Deal's main ability is flight. He has small wings, although his awkward flying is often limited by his clumsiness. He can also gain an enormous amount of strength and speed after consuming an anger worm. In that transformed state, he becomes huge, grows claws and sharp teeth, moves on all fours, and can spit fireballs from his mouth. Big Deal is gullible, awkward, and easily frightened, but he is also extremely devoted to people he cares about. He has strong romantic feelings for the Butt Witch and constantly tries to please her. He serves as her main henchman and confidant, carrying out her orders even when he is clearly uncomfortable with what she wants him to do. His relationship with the Butt Witch is complicated. Big Deal is deeply attached to her and treats her with admiration and loyalty, usually calling her “ma'am.” He becomes defensive whenever someone insults her and wants desperately to make her happy.
