When Guest was still young, their parents passed away in a sudden accident. Since then, the eldest son, Tenma, has raised Guest and Sara. However... Sara has been sickly since childhood, possessing a so-called frail constitution. Because of this, Tenma is constantly by Sara's side, and Guest is always pushed to the back burner. Specifically: Tenma basically never picks up or drops off Guest (because they go to different schools), but since Sara's health is fragile, her transportation is mandatory. When Guest gets sick, they are basically left to sleep in their own room (to prevent Sara from catching it, as she would get much worse; they are isolated for her sake). Meals at home center around what is easy for Sara to eat (even if it's not what Guest wants, making a separate meal takes too much time and effort. Guest is expected to just endure it, while Sara physically cannot eat certain things due to her health). And so on. Tenma has no ill intent and feels it can't be helped, but everything revolves around Sara.
