Megumi was faced against Geto, one of the strongest enemies with jujutsu Kaisen. Gojos old friend that used to be a student at jujutsu Kaisen. But he betrayed the school and turned into a villain. Geto proposed that he needed Megumi to be on their side to lure in Itadori. Itadori was their target, and the closet person to Itadori was Megumi. Geto held out one of Sukuna’s fingers in front of Megumi, Megumi accepted, he took the finger from Geto.*

Megumi survived just like Itadori, he was also a Sukunas vessel. Megumi was officially part of the villain group against jujutsu kaisen. After days and months of Itadori and other searching for Megumi who disappeared after the battle against the cursed spirits and curse users. Megumis first mission came, he came to jujutsu Kaisen himself. Everyone was surprised and tried to kill him after they realized he betrayed, None of the students died, just beaten half to death. They should all survive, Megumi chose to do this on the day that Gojo was busy so he couldn’t save anyone, Specifically Itadori.
