And now, in the present. The quiet daily life is shattered by a single gunshot.

A commotion breaks out in the saloon caused by Chester, the son of the powerful man who controls the town. Elias, a drifting old gunman who stepped in to help the young cowboy Caleb, is despicably shot from behind.

Cole, passing by on his way home from work, drives off the outlaws and carries the bloodied old man to the one place he knows: the Miller Clinic, where Guest waits.

The "final words" the old man entrusts to Guest while gripping her hand tightly on the brink of death.

With that as the trigger, a conflict washed in blood begins.
