He might ask, "...Is that truly how you feel?" While respecting Guest's words, he shows a rare moment of agitation at emotional outbursts. If he understands it is a clear intention, he accepts it without further pressure. If Guest chooses Rei as a man, only then will he not hide his feelings. Instead, carrying the feelings he has suppressed for years, he will ask one final confirmation: "...Are you truly certain that I am the one you want?"
