SCP-113 is an anomalous object resembling a piece of polished red jasper. When this object comes into contact with the bare skin of an organic organism possessing sex chromosomes, all biological sex-related characteristics of the body are altered over a period of approximately one minute. The transformation process consists of four stages. First, an immediate chemical mutation occurs at the cellular level upon contact. This is followed by the emission of electromagnetic waves throughout the body, causing nausea and pain. Over the next minute, the cellular structure undergoes rapid transformation accompanied by extreme agony; subjects in poor health may expire during this stage. Finally, the object detaches from the subject as the transformation completes. In most cases, the result is a complete transition to the opposite biological sex, with sex chromosomes and primary/secondary sex characteristics altered accordingly. Gender identity is generally maintained, but in cases of non-standard sex chromosomes (such as intersex individuals), the results are unpredictable and appear to be partially influenced by the individual's gender identity. Some subjects exhibit psychologically positive or negative reactions. In certain animals, the results can be fatal or lead to abnormal biological changes.

No living organisms are to be exposed to SCP-113 without prior approval. Personnel exposed to SCP-113 are to be kept under medical observation for seven days.

The gender transformation process occurs in the following four stages:
Stage 1: Duration approximately 0.2 seconds. SCP-113 bonds with the cells it touches and initiates an unidentified chemical mutation. This process causes tissue damage similar to a mild burn, and SCP-113 cannot be removed from the subject until all stages are complete.
Stage 2: Duration approximately 20 seconds. SCP-113 emits low-intensity electromagnetic waves that travel through the subject's body. The subject experiences nausea, vomiting, and a stinging sensation throughout the entire body.
Stage 3: Duration approximately 60 seconds. During this stage, the subject's cellular structure undergoes temporary transformation. Cellular changes range from forms unrecognizable as human to unique partially-differentiated stem cells. During the final 20 seconds of Stage 3, the subject experiences intense stimulation of all sensory nerves, describing the process as extremely painful. Subjects in poor health may die from shock during this stage.
At the conclusion of Stage 3, the subject's biological sex is permanently altered. In standard cases, the subject's biological sex is converted to the opposite sex. All primary and secondary sex characteristics change accordingly.
Stage 4: SCP-113 detaches from the subject and becomes inactive.
Subjects with sex chromosomes atypical for their species (e.g., intersex humans) are affected by SCP-113 in unpredictable ways. In human intersex subjects, gender identity appears to influence the outcome. Such subjects may remain unaffected, change into a standard male or female body (with corresponding chromosomal changes), or experience other results. The change usually aligns with the subject's gender identity, or at least partially aligns if the identity is non-standard. Investigation is ongoing into whether SCP-113 alters its effects based on the presence of a non-standard gender identity.
The gender identity of human subjects is generally not altered by SCP-113. For individuals with non-standard gender identities (mostly those whose pre-exposure biological sex did not match their identity), the physical changes usually result in positive psychological outcomes. Conversely, individuals with standard gender identities (males/females whose pre-exposure sex matched their identity) exhibit negative psychological reactions. This appears to be a natural psychological response rather than an anomalous effect of SCP-113.
