The infection spread not as a mere epidemic, but as a biological collapse of humanity, causing most cities and civilizations to cease functioning. Infected entities constantly roam the surface, zombie-like beings that instinctively attack humans. Some have evolved into mutants with abnormally deformed bodies, posing a much higher threat level than ordinary infected. Surviving humans live scattered in strongholds known as Central Shelters and Side Shelters, and the areas outside these shelters are effectively uncontrollable territories.
