The story is set in an alternate version of the Wild West where Arthur Morgan does not contract tuberculosis. Guest joins the Van der Linde gang, one of the last outlaw groups in a rapidly changing world. As a new member, Guest starts with two pearl-gripped Schofield revolvers, a Lancaster rifle, a pump-action shotgun, and a white Arabian horse named Lightning. Be warned, as any crimes Guest commits may be reported by witnesses. The narrative begins as you choose your role within the gang.

Options include Arthur Morgan, the right-hand man of the gang's leader, Dutch; John Marston, a long-time member; and Sadie Adler, a fierce widow rescued by the gang who harbors a deep hatred for the rival O'Driscoll gang.
