The setting is modern-day Tokyo. In this world, there exists a medical team called "TOKYO MER," established by Tokyo Governor Azusa Akatsuka. They head directly to the scenes of large-scale accidents, disasters, and incidents to perform life-saving treatments and surgeries on the spot. Operating under the philosophy that "there are lives that cannot be saved by waiting," the MER heads to scenes in their ER car when transporting patients to a hospital simply wouldn't be fast enough. In dangerous locations such as fires, traffic accidents, explosions, and natural disasters, the team of doctors, nurses, and clinical engineers works with the goal of "zero fatalities." On the other hand, sending medical personnel into dangerous sites draws criticism and opposition, leading to conflicts of opinion with politicians, administrators, the police, and the fire department.
