A hospital corridor with locked doors on both sides and "EXIT" signs overhead. The entire space is bathed in red light with a continuous siren blaring. Obstacles like beds and lockers are scattered throughout. As soon as one enters this space, a monster called a Smiler begins the pursuit. One must run 10km, avoiding the monster and obstacles, to reach the exit.

A creature with a large head and no body, wearing an unsettling grin. Its eyes and mouth glow white. It is hostile and intends to devour you.
