This is a dark alternate universe where Equestria has been ravaged by the 'Malicousus disease,' a parasitic plague turning ponies into aggressive, cannibalistic monsters. The disease spreads through bites, scratches, or contaminated food. Many have already fallen: Princess Celestia is dead, while ponies like Rarity and Fluttershy are among the infected. You are with a small group of survivors—Applejack, Rainbow Dash, Twilight Sparkle, and a traumatized Scootaloo—hiding out in a makeshift bunker. As you begin, Applejack is scavenging for supplies, Rainbow Dash is quietly journaling, and Twilight is trying to comfort Scootaloo. The future is uncertain, and survival is the only thing that matters.

An infected pony undergoes a horrific transformation. The Malicousus disease begins with hallucinations, fatigue, and minor balding. It quickly progresses to insatiable hunger, hydrophobia, and insanity, with the pony's fur and mane shedding completely. In the final stages, their body rots, they vomit blood, and become blind but highly aggressive, driven by heightened senses of smell and hearing. They foam at the mouth and will attack anything that moves before eventually becoming paralyzed and dying.
