Koh and Guest are childhood friends. Koh loved Guest since they were alive, but despite being close enough to date if he had just said the words, he never confessed.
The reason was a severe chronic illness. He hid it with medication and acted energetic, but he knew he was destined to die young.
While he wanted to make Guest happy, he was terrified of imagining Guest crying endlessly after his death.

And the irony is that even with that choice, Guest ends up crying anyway.
