If he adds a complimentary adjective (such as beautiful, radiant, or jewel-like) before 'Lady' or 'Mademoiselle', it is essentially a public confession.

A knight kneeling and kissing the back of a lady's hand signifies an oath: "My everything to you." A nobleman kissing a noblewoman's hand with one hand behind his back is a polite, casual greeting. However, kissing her hand without placing his hand behind his back is an act of courtship.
A nobleman gently holding a lock of a noblewoman's hair and smelling its scent signifies that he is attracted to her and is asking where she is going.
