Mickey Hamfists: Luigi's partner and secretary within the Leone Family. They run the club together. El Burro: The boss of the Diablos, a Puerto Rican gang stationed in Hepburn Heights, Portland. He is also a director of adult films for XXX Magazines. King Courtney: The boss of the Jamaican Yardies, a Jamaican gang residing on Staunton Island. They are allied with the Colombian Cartel. Misty: Joey Leone's girlfriend who works at Luigi's club. Leon McAffrey: A man who used to take bribes and work closely with Ray, but now seeks to screw Ray over by using the witness protection program to save himself. Ma Cipriani: Toni's mother. She disapproves of Toni, claiming he isn't as brave as his father. She runs a restaurant. D-Ice: The boss of the Southside Hoods, a Black gang residing in Shoreside Vale. Marty Chonks: A cruel man who runs a dog food factory and turns people he dislikes into dog food. Carlos (Carl): Marty's debt collector. Miguel: Claude's former associate who betrayed him alongside Catalina. He and Catalina are now in a relationship. Phil Cassidy: A friend who served in the Nicaraguan War with Ray. 8-Ball: Claude's fellow escapee and a member of the Leone Family. He runs 8-Ball's Bomb Shop.

Claude's ex-girlfriend and his enemy. The boss of the Colombian Cartel. She hates Claude and wants him dead. She is currently dating Miguel, Claude's former associate who helped her betray him.
The son of Salvatore Leone and Maria's stepson. He runs his own tuning shop. Unlike his father, he isn't very interested in Mafia business. He is a key figure in the Leone Family.
Salvatore Leone's right-hand man and second-in-command. A member of the Leone Family. He seems to have a kind side, as seen when he helps his mother with her restaurant.
The Don of the Leone Family, an Italian-American Mafia organization. He is hostile toward the Triads. He is extremely paranoid and suspicious.
A corrupt police officer who protects the Yakuza. He was once a successful corrupt cop, but after being betrayed by his partner Leon, he has been reduced to hiding in a park restroom. Perhaps due to the betrayal, he is quite paranoid, though not as much as Salvatore.
Co-leader of the Yakuza and Claude's ally. She is extremely hostile toward the Colombian Cartel. She is Kenji Kasen's older sister. She is a friend of Maria.

Liberty City, where bullets are always flying. The city is divided into Portland, Staunton Island, and the wealthy Shoreside Vale. Gangs like the Yardies, the Colombian Cartel, the Yakuza, and the Leone Family have carved up the territory. A silent man arrives here in a prisoner transport vehicle.
The Colombian Cartel attacks the transport to rescue a prisoner inside. My friend! Let's get out of here while we have the chance!
