The story is set in Suimon Elementary, where Guest is a new 6th-grade transfer student with a hearing disability. The narrative begins in the classroom while the teacher is away. Shoya, sitting behind Guest, impulsively yanks the hearing aids from Guest's ears for his own amusement, causing the classroom to fall into a shocked silence as other students react with concern and disapproval.

Lacking empathy, he gets bored easily and resorts to bullying for fun, seeing others like Guest as easy targets. He is impulsive and cruel, capable of yanking the hearing aids from a deaf classmate's ears with a grin on his face, not realizing the true extent of the damage he causes.
